Online Forms and Cancellation Policy

Beyond Chiropractic offers our patient form(s) online so you can complete it in the convenience of your own home or office.

  • Fax us your printed and completed form(s) or bring it with you to your appointment.

Chiropractic - New Patient Forms:

This let's us know the history and current state of your health. What questions, concerns, goals, regarding wellness can we help you with? Let us know! Please fill out the New Patient Health History Form if you are visiting the clinic for either chiropractic or nutritional services.

Nutra-Metabolic Evaluation - New Patient Forms:

1. Nutritional Assessment Questionnaire - Required

This lets us know the history and your current state of health and what your specific challenges.

When filing out the forms below it is imperative that you give a detailed history.  This is often turned in on a separate typed sheet of paper.   All forms and relevant medical test results that have been done with in the last 6 months MUST be in the office at least two business days before your initial exam.

All New Nutra-Metabolic Evaluation Patients should come in fasting for the Body Composition Analysis.

Cancellation Policy:

We understand that life happens and there may be times you will need to cancel your appointment. Beyond Chiropractic has a 24 hour cancellation policy. In the event that you are unable to keep your appointment please call our office the previous business day at least 24 hours in advance. There is a $50.00 cancellation fee for each service scheduled when less than 24 hours’ notice is provided. When scheduled services are less than $50.00, the full price of the service will be charged. Please call on Friday before 5:00 pm when cancelling appointments for the following Monday.

Cancellation fees for new wellness visits: New wellness client evaluations require an extensive block of time on our clinic schedule. The time required is approximately one hour and thirty minutes. . In the event that you are unable to keep your appointment please call our office the previous business day at least 48 hours in advance. The cancellation fee for this service when adequate notice is not provided is $150.00 as it prevents other clients from being able to schedule on that day.

We thank you in advance for your understanding and consideration of Dr. Way and staff who are preparing for your visit, as well as other clients who are waiting for notices of openings in the schedule for their care.
